Ordering a SIBO Test

Ordering process

If you are a patient interested in doing the SIBO testbring this brochure to your doctor. A doctor will determine if SIBO testing is appropriate for a patient. We must have an order from a doctor to give patients a test kit.

If you are a provider interested in offering the test to your patients – email your request using this form. After submitting your information the ordering process and order form will be emailed to you. The ordering process involves the provider faxing an order and directing their patient to the online payment portal. Once we receive the order and payment the kit will be shipped directly to the patient. There is also an option to have the patient do the collection at our clinic.

How much does the test cost?

SIBO kits are $180 and can be mailed directly to the patient to be completed at home or patients can schedule an in-house appointment with a SIBO Technican.

Payment / Insurance

When a provider places an order for the test they will direct the patient to an online payment portal where patients can securely pay via credit card (Visa, MasterCard, Discover) or by check. As part of the payment process the patient will have the option to submit insurance information. Please note, depending on the insurance carrier, kits will be charged in advance and a refund will be issued when the insurance payment reconciles.

Returning your SIBO Kit to the Lab

A return shipping label will be included in kits dispensed in the United States. If there is no return label the completed kit can be dropped off at the NCNM Lab anytime the clinic is open (see hours) or mailed to the following address:
