Ordering a SIBO Test

Ordering process

If you are a patient interested in doing the SIBO test – bring this brochure to your doctor. A doctor will determine if SIBO testing is appropriate for a patient. We must have an order from a doctor to give patients a test kit.

If you are a provider interested in offering the test to your patients – email your request using this form. After the provider is set up in our system an order form will be filled out and faxed to the Center. Upon receipt of the form the patient will be charged for the kit and it will be shipped directly to the patient. If the patient does not feel comfortable collecting the samples at home, there is an option to schedule collection in our lab by a trained professional.

How much does the test cost?

Option 1: $155 to have the kit mailed directly to the patients home. Please note that there is no shipping charge and a return shipping label will be included in each kit for those kits returning from the United States.

Option 2: $180 to do the procedure in our lab. Please call 503.552.1980 to arrange an appointment.

Payment / Insurance

Payment can be made by credit card (Visa, MasterCard, Discover) or by check. Indicate which form of payment is preferred on the order form provided by the provider. As part of the ordering process the patient will have the option to fill out a courtesy billing form for insurance. Please note that the courtesy billing form must accompany the original order. Please note, kits may be charged for in advance depending on the insurance carrier and a refund will be issued when the insurance payment reconciles.

Returning your SIBO Kit to the Lab

A return shipping label will be included in kits dispensed in the United States. If there is no return label the completed kit can be dropped off at the NCNM Lab anytime the clinic is open (see hours) or mailed to the following address:
