Ordering process
If you are a patient interested in doing the SIBO test – bring this brochure to your doctor. A doctor will determine if SIBO testing is appropriate for a patient. We must have an order from a doctor to give patients a test kit.
If you are a provider interested in offering the test to your patients – Please note that the SIBO kits we dispense contain Lactulose which is a legend drug regulated by the FDA. Kits can only be ordered by practitioners who have prescribing rights for legend drugs. This right is conferred through the practitioners’ state licensing board and is clearly defined in the scope of practice. If you are unsure of your prescribing rights contact your local licensing board.
Once you have determined you have prescribing rights, follow this link to fill out the Provider Registration Form. After submitting your information the ordering process and order form will be emailed to you. The ordering process involves the provider faxing an order and directing their patient to an online payment page. Once we receive the order and payment the kit will be shipped directly to the patient.
How much does the test cost?
SIBO kits are $199 and will be mailed directly to the patient to be completed at home.
Payment / Insurance
When a provider places an order for the test they will direct the patient to an online payment portal where patients can securely pay via credit card (Visa, MasterCard, Discover) or by check. As part of the payment process the patient will have the option to submit insurance information. Please note, depending on the insurance carrier, kits will be charged in advance and a refund will be issued when the insurance payment reconciles.
Returning your SIBO Kit to the Lab
A return shipping label will be included in most kits dispensed in the United States. If there is no return label the completed kit can be dropped off at the NUNM Lab anytime the clinic is open (see hours) or mailed to the following address:
